For Music + Language Classes: Summit Speech and Language Therapy, PLLC (“Summit”) is providing a class for you and your child focused on music and language. Summit is not providing Speech Therapy services during the class. If you are interested in Speech Therapy services, please reach out to Summit after the class. 



I grant Summit permission to take and to share photographs and/or video recordings of my child for marketing purposes. I release Summit from all claims, damages, and liabilities that come out of connection with the use or distribution of photographs and video recordings of my child with Summit. If I do not consent to this, I will speak with Summit prior to the class.



I agree to pay the full class fee, or to cancel or reschedule any class at least 48 hours in advance. I agree that I am responsible to pay the entire fee charged to me by Summit if cancellation occurs less than 48 hours prior to my appointment. This is necessary because a time commitment is made to me and is held exclusively for me. 

I understand and am informed that participation in this class may have some risks. I understand that I have the right to ask about these risks and have any questions answered prior to the class.



I agree to indemnify, and hold harmless Summit from all claims, suits, and actions of any nature resulting from or arising out of, or related in any way to the activities in which I participate with Summit, or to the subject matter of this agreement.
